Request for bank documentation to process your refund

In order to proceed with your refund request, we kindly ask that you provide one of the following bank documents:

A letter from your bank that includes your account name, BSB, and account number.
A redacted bank statement showing only the top portion with the relevant bank details, issued by your bank.
A deposit slip that includes your account information.

Safe Transport Victoria collects this information to verify your bank account details and process your request for a refund. Your personal information provided in this form will be handled in accordance with Safe Transport Victoria’s Privacy Policy and the Privacy and Data Protection Act 2014 (Vic).

We will not share this information with any other individual or organisation unless required or authorised by law.

If you do not provide the required information, we will not be able to verify your bank account details and process the refund.
