CPV duty holders are required to report certain incidents under the Commercial Passenger Vehicle Industry Act 2017.

Notifying us of fatalities, serious injuries and incidents that expose people to safety risks helps us monitor safety and identify trends across the commercial passenger vehicle industry.

From June 2026, our reporting form looks a little different and we now require you to choose an incident type in the form. There is no need to log into a portal to submit an incident.


How do I choose the incident type?

When submitting a notifiable incident report, select the incident type that best matches what happened.

If you’re unsure, choose the closest match and provide as much detail as possible in your report.

When to select this

Select this incident type if your bus or vehicle has collided with something.

This includes

  • Another vehicle (car, bus, truck or motorcycle)
  • A pedestrian or cyclist
  • Infrastructure (for example, a pole, barrier or building)
  • Road debris or loose objects
  • Overhead structures (for example, a bridge or sign)

When to select this

Select this incident type if a passenger is injured or put at risk as a result of the journey or vehicle operation.

This includes

  • Slip, trip or fall
  • Injury caused by braking, acceleration or turning
  • Boarding or alighting incidents
  • Passenger caught in doors
  • Unsafe pick-up or drop-off (CPV)

This does not include

  • Incidents involving intentional harm or threats (select Intentional harm / security)
  • Sudden illness or medical conditions (select Medical episode)

When to select this

Select this incident type if the incident involves inappropriate, unsafe or non-compliant behaviour that does not involve intentional harm or threats.

This includes

  • Driver misconduct
  • Passenger misconduct
  • Anti-social behaviour

This does not include

Incidents involving assault, threats or violence (select Intentional harm / security)

When to select this

Select this incident type if someone has intentionally caused harm or posed a security risk.

This includes

  • Physical or sexual assault
  • Threats or intimidation
  • Aggressive or violent behaviour
  • Theft or vandalism
  • Any incident where someone intentionally causes harm or fear

When to select this

Select this incident type if a driver, passenger or another person experiences a sudden illness or medical event.

This includes

  • Driver medical episode
  • Passenger medical episode
  • Illness affecting another person involved

When to select this

Select this incident type if the incident involves a vehicle fault or malfunction.

This includes

  • Brake, steering or tyre failure
  • Door malfunction
  • Engine or electrical fault
  • Wheel dislodgement
  • Other mechanical issues affecting safe operation

When to select this

Select this incident type if the incident involves fire or another hazardous event.

This includes

  • Fire
  • Explosion or implosion
  • Smoke or overheating

When to select this

Select this incident type if the incident involves unsafe driving without a collision.

This includes

  • Vehicle leaving the roadway
  • Loss of control
  • Significant road rule breach

When to select this

Select this incident type if a driver is affected by drugs or alcohol, or is unfit for duty.

This includes

  • Positive drug or alcohol test
  • Driver assessed as unfit before or during service

What types of incidents must be reported?

Notifying us of fatalities, serious injuries or incidents which expose people to risks to their safety allows us to monitor safety and causes of incidents in the commercial passenger vehicle industry.

The following incidents must be reported:

  • The death of any person
  • The serious injury* of any person (this includes but is not limited to incidents that require emergency medical services assistance)
  • Attendance by police
  • Attendance by a health professional

* if you’re unsure if it’s a serious injury then it’s best to report the incident.

Who must report?

The following duty holders, as defined in the Commercial Passenger Vehicle Industry Act 2017, are required to report a notifiable incident:

  • Booking service provider
  • Commercial passenger vehicle owner
  • Commercial passenger vehicle driver
  • Those with control over the provision of commercial passenger vehicle services
  • Supplier of equipment to commercial passenger vehicles

It is an offence not to report a notifiable incident. The failure to do so may result in compliance action by Safe Transport Victoria.

When do I need to report an incident?

If you are a duty holder, you must notify us within ten business days of becoming aware that an incident has occurred.

Can I report on behalf of someone else?

Drivers, vehicle owners and equipment suppliers who experience an incident, are encouraged to notify their Booking Service Providers (BSP) of the incident, and the BSP can notify us on their behalf.

Passengers and bystanders can’t report a notifiable incident but are encouraged to provide information on any incidents through the Safe Transport Victoria feedback and complaint form.
